- Right to Request Restrictions: You may ask us not to share certain information; we are not required to agree to all requests.
- Right to Request Confidential Communication: You may request how or where we contact you, in writing.
- Right to a Paper Copy of This Notice: You can request this anytime, even if you’ve agreed to receive it electronically.
- Right to Electronic Records: You can receive your PHI in electronic form.
- Right to Breach Notification: You will be notified if your PHI is improperly disclosed.
CHANGES TO THIS NOTICE
We may change this notice at any time and apply changes to existing and future Health Information. The current version will be posted on our website.
COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with us or the Department of Health and Human Services. No retaliation will occur.
